The success of a season should not be measured solely on wins and losses but more importantly by the development of the players as individuals and as a team.
Below is the Sacred Heart Coaches’ Evaluation Form by which the Booster Organization solicits your input on the performance of the coach(es) of your son’s or daughter’s team during this most recent season. The data you provide will be compiled and forwarded to the head coach of each respective team. The coaches will not see the original forms and the data will be “scrubbed” of any specific references to players or parents by name, so please be as candid as you desire. Your anonymity will be protected.
